Industrial Wastewater Survey

The City of Denison is required by federal regulations to identify and locate all possible Industrial Users that may be subject to the federally mandated pretreatment program. The city continuously surveys industries and may request periodically that industries fill out all required survey forms. Failure to submit the form at the city's request may result in a violation.

Please download the survey from the supporting documents, then fill out the form and attach the completed form before submitting.

Submittal options:

        -Download survey as a PDF, fill out PDF, save, attach file

        -Open survey in browser, fill out survey, download "with your changes", attach file

        -Fill out form manually, scan in copy, attach file
