Step One

The Big Event Volunteer Application
Please read before completing The Big Event Volunteer Application:
If you are applying on behalf of your group, only the designated Group Leader should complete the following application.
Group Leaders must be 18 years of age or older.
Student or youth groups MUST be led by a school faculty member or approved adult leader.
Limit is 10 team members per group; however, if your organization is volunteering a larger group, please submit a new form for each additional group.
In addition to submitting The Big Event Volunteer Application, the Group Leader is responsible for submitting the Group Release and Waiver of Liability, which requires the signature of each team member (scanned and emailed to bigevent@denisontx.gov, or returned to City Hall at 300 W. Main Street).
The form will be emailed to you once you submit the online application.

Disclaimer

As a volunteer for the City of Denison:
I agree to accept full responsibility for myself and each of the participants in my group to serve in a volunteer capacity with the City of Denison. I understand that we will be expected to meet all the requirements of the volunteer position, including attendance, and adherence to City of Denison policies and procedures.
In consideration of the City of Denison allowing me to participate as a volunteer, and that possible injuries could occur as a result of that participation, I release City of Denison officials, employees, agents, and volunteers from any and all injuries and damages whatsoever arising from participation in the event. I, my heirs, and representatives, agree to indemnify, save, and hold harmless the City of Denison, it's officials, employees, and agents from any and all claims made my me or my insurer for injuries or damages related to this event.
As a volunteer, I will place safety and well-being first, and represent the City of Denison in a professional manner that presents a positive image to the community.
Authorization to Use Image: Volunteers hereby consent to their photograph being taken, or image recorded by the City of Denison in connection with its Big Event. Furthermore, I authorize the use, copyright, or publication of my name, image, or voice as may be captured by photograph or recording while attending or participating in the Big Event, in any medium, for any purpose, including illustration, promotion, marketing or advertisement.
I also agree to access confidential information to the minimum extent necessary for my assigned volunteer duties and only disclose such information to persons authorized to receive it.
As a volunteer team leader I affirm that:
I will follow the guidelines communicated to me by the department as they pertain to my volunteer activities.
I agree not to consume, use, possess, or be under the influence of any drug/alcohol products while volunteering.
I understand that any conduct that disrupts, diminishes or otherwise jeopardizes public will result in dismissal.
I understand that my volunteer assignment with the City of Denison may be terminated at any time.
I understand that I am a volunteer for the City and not considered an employee in any manner nor am I eligible for benefits or rights of an employee of the City.
I certify that I have read this release thoroughly, that I understand its terms and conditions, and that I make this release and waiver voluntarily and have not relied upon any representations made by the City of Denison, its officers, agents, employees, or volunteers.
Electronic Signature Agreement
